Because Alzheimer's disease cannot be cured and is a chronic degenerative disease, it may place a heavy emotional toll on the caregivers who most often are close relatives. The early stages of Alzheimer's disease may manifest themselves up to five to 10 years prior to a clinical diagnosis. The early symptoms can include changes in attentiveness, reductions in the ability to think abstractly and a decreasing ability to be flexible in day-to-day activities. Perhaps the most common early symptoms relate to difficulties in learning new things or remembering recent events. These symptoms often are attributed to normal aging. As the disease progresses, there are increasing problems with learning and memory that start to impair the ability to carry on day-to-day activities. When this occurs, the diagnosis of Alzheimer's is generally made.

The middle stage of Alzheimer's is characterized by an inability to perform most of the necessary activities of daily living. At this time, the individual with Alzheimer's may start to forget who their loved ones are, start losing their long-term memory, begin losing their coordination - making them susceptible to falls, and lose the ability to read or write. Perhaps, most disturbing to loved ones are the behavior changes that begin. These changes include irritability, outburst of anger or aggression, wandering and resistance to caregivers. About 1/3 will begin to have delusions. An interesting phenomenon is Capgra's syndrome, where the Alzheimer patient believes that his or her caregiver has been replaced by an imposter.
